Sanders Maturity Score
1) Description of Measurement
The Sanders Maturity Score is a hand radiograph–based skeletal maturity system developed to more precisely estimate remaining growth potential in children and adolescents, particularly those with scoliosis. It evaluates the pattern of epiphyseal and physeal maturation in the hand and wrist and directly tracks progression through the peak height velocity (PHV) period.
2) Instructions to Measure
Obtain a standard PA radiograph of the left hand and wrist.
Evaluate the morphology and closure status of:
Phalangeal epiphyses
Metacarpal epiphyses
Distal radius epiphysis
Assign a Sanders stage (1–8) based on specific epiphyseal shape changes, capping, and fusion patterns (please see the image).
Staging follows a sequential maturation pathway, moving from open physes with minimal epiphyseal enlargement to complete physeal closure.
3) Normal vs. Pathologic Ranges
Sanders stages represent normal skeletal development phases. Their importance lies in growth-risk stratification.
Stages 1–2:
Prepubertal growth
Low immediate progression risk
Stages 3–4:
Early puberty and onset of peak height velocity
Highest risk of scoliosis progression
Stage 5:
Late peak height velocity
Growth rate slowing, but progression risk persists
Stages 6–7:
Decelerating growth, approaching maturity
Reduced progression risk
Stage 8:
Complete skeletal maturity
Minimal to no risk of curve progression

The Sanders Maturity Score is valuable as it indicates peak height velocity, the best predictor of rapid scoliosis progression. The Risser sign, reflecting later skeletal maturity, may underestimate growth potential early in puberty.
