Chiari Malformation
1) Description of Measurement
Chiari malformations are congenital abnormalities of the posterior fossa characterized by caudal displacement of hindbrain structures through the foramen magnum. MRI-based measurements assess the degree and pattern of descent of the cerebellar tonsils, vermis, brainstem, and associated posterior fossa anatomy. These measurements help distinguish Chiari subtypes, evaluate cerebrospinal fluid obstruction, and identify associated pathologies such as syringomyelia, hydrocephalus, or spinal dysraphism.
Type I: Inferior displacement of cerebellar tonsils only
Type II (Arnold–Chiari): Herniation of cerebellar tonsils, vermis, and medulla
Type III: Herniation of cerebellar and/or brainstem structures through an occipital skull defect (encephalocele)
2) Instructions to Measure
Obtain midline sagittal MRI of the brain and craniocervical junction (T1 or T2).
Identify the basion–opisthion line (McRae line) spanning the foramen magnum.
Identify the foramen magnum reference line drawn between the basion and opisthion.
Measure the vertical distance from this line to the lowest point of:
Cerebellar tonsils (Type I)
Vermis and brainstem structures (Type II)
Evaluate posterior fossa morphology:
Size and crowding
Brainstem configuration
Assess for associated findings:
Syringomyelia on sagittal and axial spinal MRI
Hydrocephalus (ventricular enlargement)
Myelomeningocele or encephalocele
In suspected Type III, use CT to characterize the osseous skull defect.
3) Normal vs. Pathologic Ranges
Normal: ≤3 mm below foramen magnum (some consider up to 5 mm borderline, especially in children)
Pathologic: ≥5 mm tonsillar descent below the foramen magnum

5) Other info....
Type I Chiari malformation is the most common anomaly; 1 in 1,000 live births. Type II Chiari malformation is less common. Type III is even rarer. Symptoms do not correlate perfectly with millimeters of tonsillar descent. Posterior fossa size, not just tonsillar position, plays a major role in pathophysiology.
